Helpful items to have before you start:

Your drivers license

Your child's preferred hospital and primary physician contact information

Immunization records

Contact information for anyone who will be authorized to pick up your children

Contact information for anyone who will be listed as an emergency contact

A credit card for the $99 enrollment fee (this isn't needed for CCAP approved children)
